MEDICAL SOCIAL SERVICES

At the NWVNA, social workers work with our nurses, physical therapists, occupational therapists, speech therapists, nutritionists, and home care aides to assist patients and families.

Social workers help people overcome some of life’s most difficult challenges, which can include everything from: physical illness, discrimination, abuse, addiction, to divorce, loss, unemployment and financial difficulties. Social workers help prevent crises and counsel individuals, and families so they can cope more effectively with the stresses of everyday life.

 

By working closely with the social workers in our community we are able to help guide patients and their families through challenging decision-making that arises when a home no longer meets a patient’s needs or changes need to be made in the home to accommodate a patient's health circumstances.

Medical Social Services:

  • Helping patients adjust to an illness

  • Facilitating linkage to and assistance with necessary community resources needed, including:

    • Long-term care education and planning

    • Elder care services, including Meals on Wheels

    • Prescription, fuel and food assistance

    • Transportation

    • State and federal programs including Medicare, Medicaid, and Social Security
